We are here to help with up to 12 weeks of support at no cost to you.
We know the first few months with a baby can be an overwhelming time for the entire household. That’s why we offer a 12-week program that provides support for your newborn and your entire family.
Family Connects North Carolina; Southeastern Region is a community-wide in-home nurse visiting program available to families with newborns in Cumberland, Hoke, or Robeson Counties.
Eligible families who sign up will receive up to 12 weeks of support, including an in-home or virtual* visit by a specially trained registered nurse from our nurse partners at Carolina Collaborative Community Care (4C). The nurse visit happens around 3 weeks after birth. After the nurse visits, you will receive post-visit follow-up and support until your baby is 12 weeks old.
* Virtual visits are conducted via telephone, web-based telehealth, etc.
