Step #2 – Complete Contact Form and Upload Application and Documents for Full-Time Students

The Contact Form will allow you to upload all your documents in one place so they stay together with your application.


Required Documents* (full time student)


Proof of Child’s Date of Birth

We need a copy of the child's birth certificate (does NOT need to be a certified or official copy).

Proof of Residence

We require NC issued photo identification or NC driver's License.

Proof of Enrollment

If you are a High School Student:

  • Referral from your school counselor
  • Copy of class schedule
  • Copy of previous semester's grades

If you are a College Student or in a Training Program:

  • Referral letter from DSS
  • Proof of payment for classes/program
    • Financial Aid, Pell Award, and/or Scholarship Award letter
    • Cash Receipt
  • Copy of class/program schedule
  • Copy of previous semester's grades

Financial assistance is based upon available funding; as a result, we may maintain a waiting list. if you are placed on a waiting list, one of our caseworkers will contact you as funds become available.


Required Documents* (Part-time or job seeking)

Proof of Child’s Date of Birth

We need a copy of the child's birth certificate (does NOT need to be a certified or official copy).

Proof of Residence

We require NC issued photo identification or NC driver's License.

Proof of Employment or Job Seeking

You must include the following:

  • Referral letter from DSS
  • One of the following for proof of income:
    • Pay Stubs for a complete month (if paid weekly – submit 4 consecutive pay stubs; if paid twice a month – submit 2 consecutive pay stubs; if paid monthly – submit at least 1 full month’s pay stub), OR
    • Employer written statements signed by the employer (on letterhead, if available), OR
    • Employment Verification Form (Click to download)
  • Verification letter that you are receiving benefits from the Supplemental Nutrition Assistance Program (i.e., EBT, food stamps, etc.)

Required Documents* (Crisis)

Proof of Child’s Date of Birth

Proof of Residence

We require NC issued photo identification or NC driver's License.

Proof of Crisis

You must have a referral letter from a community agency, documenting the crisis, on the organization's letterhead and signed by a representative of the organization.


Registration Fee*

  • Referral letter from DSS

How do I know if I might be eligible for Child Care Assistance?

Financial assistance may be available on a temporary basis to parents who:

  • Work part-time or are actively seeking employment
  • Attend school or an approved training program (must be attending full time)
  • May be experiencing a crisis (eligibility for assistance is determined on a case by case basis)
  • Has a child with documented developmental needs

For your family to qualify for financial assistance, you must:

  • Be a Cumberland County resident
  • Have a child under five years old (five years old cannot be enrolled in Kindergarten)
  • Meet income limits (income eligibility is based on 75% of state median income)
