It is a voluntary program to assist families with at-risk or special needs children to obtain needed services. Support is provided to ensure that children have maximum opportunity to reach their developmental potential. Services include developmental screening and monitoring, physical assessments, preventive health education, developmental activities, medical/health care maintenance, resource referrals, information on child development and coordination of services. Services are provided in the home or at a site of choice by highly skilled nurses and social workers.
